Working together to support the whole person.

We work alongside GPs, allied health professionals, support coordinators, rehabilitation providers and other services involved in a person’s care.

Practical and psychosocial support

Social work can help address practical and psychosocial factors affecting wellbeing, participation and engagement with care. With appropriate consent, we can communicate with relevant treating and support providers to contribute to coordinated care.

  • Psychosocial assessment and wellbeing support
  • Relationships and family pressures
  • Isolation and community connection
  • Housing and financial stress
  • Service navigation and advocacy
  • Routine, purpose and meaningful activity
  • Barriers to engagement
  • Coordination with other providers

DVA referrals

For eligible DVA clients, GPs can use an appropriate DVA allied health referral. We recognise the GP’s role in oversight of the person’s broader healthcare.
